  Clarify that foreign direct slotds's "offset" is a bit offset; change the
  name of the slot and its accessor/initarg to reflect this.
  
  Foreign effective slot definitions generally treat the location as a byte
  offset (except the slot's type is a bit or bitfield.)
  
  Fix some bugs in calculating instance size of lisp-defined classes, which
  had led to general chaos/memory corruption problems.
